In this example we are completing as an agent for a child of the deceased.
If you want to be cremated, it’s a good idea to make your wishes clear by signing a cremation declaration. This records your decision and helps ensure that the people arranging your funeral can follow your wishes, even if someone later tries to change them (see orange circled section).
